Download, complete, and submit all required forms for your athlete or to volunteer with the Upland Outlaws. All completed forms should be uploaded through your PlayMetrics account.

Download each form, complete it, and upload it to your PlayMetrics account. All three forms are required before your athlete can participate.

Mt. Baldy Physical Form
The official 2026 Mt. Baldy Conference physical form. Must be completed and signed by a licensed physician before the season begins.

The 2026 participant contract and parental consent form. Must be signed by a parent or legal guardian prior to participation.

Volunteer Forms
All coaches, volunteers, and staff working with athletes are required to complete a LiveScan background check before the season begins.

Required for all coaches, volunteers, and staff who will be working directly with athletes. Download the form, take it to a LiveScan provider, and submit your results to volunteer@uplandoutlaws.org

Submit Forms.
Documents must be submitted two ways: digital copies uploaded to PlayMetrics via the Resources tab, and physical copies handed directly to your team's assigned Athletic Director.

Download & Complete
Download the required forms above, print them, and have them completed and signed by the appropriate parties (physician, parent/guardian).

Scan or Photograph
Take a clear photo or scan of each completed form. Make sure all fields and signatures are clearly visible.

Upload Digital Copies via PlayMetrics
Log into PlayMetrics, open your athlete's profile, click the Resources tab, then click Add New Resource to upload digital copies of all required documents.
